The Integrated Aged Care Program offers Home Care packages which include Community Aged Care Packages, Extended Aged Care at Home, Dementia Packages and the Transition Care Program (TCP).
These services are funded through the Commonwealth Department of Health and Ageing and the State Department of Health (TCP). Eligibility for these services is determined by the Aged Care Assessment Services.
CACPs are low-level Home Care packages for older people, over 65 years, who are frail or have disabilities, and would be eligible for admission to hostel care but who wish to remain living at home with support.
EACHD packages are high-level Home Care packages designed to meet the needs of frail older people with dementia who experience behaviours of concern.
The TCP provides short-term support and management for older people after a stay in hospital. Participants are provided with a range of services including nursing, personal care, allied health, case management and medical support as part of an ongoing but slower recovery process while options for their longer term care arrangements are considered.
Hepburn Health Service provides up to fourteen community and residential places to people living in the Hepburn Shire. Residential TCP is provided at the Creswick Hospital and the Trentham Hostel.
